ABSTRACT

Background/Aims@#Dietary factors can aggravate the symptoms of irritable bowel syndrome (IBS). Many IBS patients try restrictive diets to relieve their symptoms, but the types of diets with an exacerbating factor are unknown. Therefore, this paper reports the results of a systematic review and network meta-analysis of randomized-controlled trials (RCTs) reviewing the efficacy of food restriction diets in IBS. @*Methods@#The MEDLINE, EMBASE, Cochrane Central Register of Controlled Trials, and Clinicaltrials.gov databases were searched until July 21, 2021, to retrieve RCTs assessing the efficacy of restriction diets in adults with IBS. Two independent reviewers performed the eligibility assessment and data abstraction. RCTs that evaluated a restriction diet versus a control diet and assessed the improvement in global IBS symptoms were included. These trials reported a dichotomous assessment of the overall response to therapy. @*Results@#A total of 1,949 citations were identified. After full-text screening, 14 RCTs were considered eligible for the systematic review and network meta-analysis. A starch- and sucrose-reduced diet and a diet with low-fermentable oligosaccharides, disaccharides, monosaccharides, and polyols (FODMAPs) showed significantly better results than a usual diet. Symptom flare-ups in patients on a gluten-free diet were also significantly lower than in those on high-gluten diets. @*Conclusions@#These findings showed that the starch- and sucrose-reduced, low FODMAP, and gluten-free diets had superior effects in reducing IBS symptoms. Further studies, including head-to-head trials will be needed to establish the effectiveness of dietary restrictions on IBS symptoms.

ABSTRACT

Background@#Risk assessment tools have been actively studied, and they summarize key predictors with relative weights/importance for a disease. Currently, standardized screening scores for type 2 diabetes mellitus (DM) and chronic kidney disease (CKD)—two key global health problems—are available in United States and Korea. We aimed to compare and evaluate screening scores for DM (or combined with prediabetes) and CKD, and assess the risk in contemporary United States and Korean populations. @*Methods@#Four (2×2) models were evaluated in the United States-National Health and Nutrition Examination Survey (NHANES 2015–2018) and Korea-NHANES (2016–2018)—8,928 and 16,209 adults. Weighted statistics were used to describe population characteristics. We used logistic regression for predictors in the models to assess associations with study outcomes (undiagnosed DM and CKD) and diagnostic measures for temporal and cross-validation. @*Results@#Korean adult population (mean age 47.5 years) appeared to be healthier than United States counterpart, in terms of DM and CKD risks and associated factors, with exceptions of undiagnosed DM, prediabetes and prehypertension. Models performed well in own country and external populations regarding predictor-outcome association and discrimination. Risk tests (high vs. low) showed area under the curve >0.75, sensitivity >84%, specificity >45%, positive predictive value >8%, and negative predictive value >99%. Discrimination was better for DM, compared to the combined outcome of DM and prediabetes, and excellent for CKD due to age. @*Conclusion@#Four easy-to-use screening scores for DM and CKD are well-validated in contemporary United States and Korean populations. Prevention of DM and CKD may serve as first-step in public health, with these self-assessment tools as basic tools to help health education and disparity.

ABSTRACT

Purpose@#This study examined the subjective frame of patient satisfaction in the comprehensive nursing care services and explored future directions by investigating subjective frame types. @*Methods@#P-Samples were collected from 30 patients who underwent the comprehensive nursing care service. To enable a forced normal distribution, 31 Q-samples were classified on a 9-point scale considering the degree of agreement. The collected data were analyzed by principal component factor analysis and varimax rotation using the PQ method program. @*Results@#The subjective frame of patient satisfaction with the comprehensive nursing care service was divided into four types. Considering the characteristics of each, the types were labeled as “the formation of relationships with the surroundings”, “autonomous decision-making”, “non-verbal empathy”, and “therapeutic support resources”. Each type can be classified into therapeutic relationship and therapeutic process categories. @*Conclusion@#This study revealed changes in expectations regarding new medical services and that the subjective frames that affect the formation of satisfaction are diverse. The study results are expected to be used as basic data to develop strategies for creating a therapeutic environment that improves patient satisfaction in the comprehensive nursing care service.

ABSTRACT

PURPOSE: Postmenopausal women are a unique population with high risk for the degeneration of muscle. The aim of the present meta-analysis was to accurately evaluate the effects of vitamin D on muscular strength in postmenopausal women.METHODS: A review was conducted using electronic databases, including PubMed, EMBASE, Ovid Medline, CINAHL complete, and the Cochrane Library from inception through 19 March 2019. Included studies were selected by two independent reviewers. The meta-analysis were performed using Review Manager 5.3 software.RESULTS: A total of nine randomized controlled clinical trials were included in this review. Vitamin D interventions led to no changes in the upper limb muscle strength (mean difference –0.16, 95% CI: –1.09 to 0.77), lower body muscle strength (standard mean difference 0.08, 95% CI: –0.11 to 0.26), and back/hip muscle strength (standard mean difference 0.06, 95% CI: -0.05 to 0.17).CONCLUSION: Pooled results from eight studies indicated that supplementation of vitamin D did not increase muscle strength in postmenopausal women. Apparently, the present review suggests that supplementation of vitamin D alone had no didn't show any beneficial effects on muscle strength in postmenopausal women.

ABSTRACT

Boxer's knuckle has become known as rare diseases caused by trauma, congenital, idiopathic, degenerative and more. Because there is no research for specific prevalence rate, the purpose of this study is to figure out the possibility of the prevalence rate of Korean early 20's who relied on data from the Internet questionnaire about the group suspected of having Boxer's knuckle by self-diagnosis after an indirect explanation for Boxer's knuckle. The positive response rate of Boxer's knuckle analyzed by a public Internet survey questionnaire contains personal information, Boxer's knuckle symptoms, location, risk factors, family history, and so on. Total of 304 respondents include 106 males (median age, 23 years; Q1, 21–Q3, 23), 198 females (median age, 21 years; Q1, 21–Q3, 23). Thirty-three patients had severe symptoms of Boxer's knuckle (dislocation) and 42 patients had mild symptoms of Boxer's knuckle (subluxation). Sex, age, type of exercise and duration of exercise, family history, etc were analyzed. It is a result that whether or not history of severe exercise and family history are statistical significances as risk factors. Fifteen (7.7%) of 196 people who do not exercise, seven (14.0%) of 50 people with light exercise, 11 (19.0%) of 58 people with severe exercise, 28 (9.5%) of 294 people without family history and five (50.0%) of 10 people with family history showed symptoms of the Boxer's knuckle. The positive response rate of the Boxer's knuckle is 10.9%. The exercise in the possibility of injury and family history are significant predictors.

ABSTRACT

BACKGROUND: We investigated the clinical results and early complications after lateral unicompartmental knee arthroplasty (UKA) using the Zimmer unicompartmental high-flex knee (ZUK) prosthesis with a minimum follow-up of 2 years. METHODS: Twenty-seven patients (30 cases) who underwent lateral UKA with the ZUK prosthesis between January 2011 and February 2014 were selected for this study. The mean age of the patients was 63.3 years at the time of surgery, and the mean follow-up was 3 years and 2 months (range, 24 to 48 months). A retrospective investigation method was used to evaluate the clinical and radiographic results with use of the Knee Society (KS) clinical rating system and plain radiography. RESULTS: The mean KS pain score was improved from 17.9 points (range, 10 to 30 points) preoperatively to 40.5 points (range, 30 to 45 points) at the final follow-up (p < 0.001). The mean KS knee score and function score significantly increased from 63.2 points (range, 48 to 70 points) and 68.6 points (range, 35 to 80 points), respectively, preoperatively to 86.0 points (range, 74 to 95 points) and 92.4 points (range, 60 to 100 points), respectively, at the final follow-up (p < 0.001). The mean range of motion of the knee was recovered from 127.1° (range, 110° to 135°) preoperatively to 131.6° (range, 120° to 135°) at the final follow-up. The mean tibiofemoral angle changed from 6.2° of valgus (range, 0.1° to 11.4° of valgus) preoperatively to 3.4°of valgus (range, 0.2° to 9.5° of valgus) at the final follow-up. The overall results classified based on the KS knee score were "excellent" in 21 cases and "good" in 8 cases. Revision total knee arthroplasty was required in one case because of consistent pain on the medial side of the knee after surgery. CONCLUSIONS: The early clinical results of lateral UKA using the ZUK prosthesis were satisfactory for improvement of pain, knee score, function score, and recovery of knee motion. Therefore, the lateral UKA could be a useful method in the treatment of osteoarthritis in the lateral compartment of the knee. Long-term survivorship remains to be confirmed through ongoing follow-up.

ABSTRACT

STUDY DESIGN: This is a retrospective clinical study. OBJECTIVES: To compare the efficacy of lidocaine and bupivacaine for the ultrasound-guided lumbar medial branch block in chronic low back pain. SUMMARY OF LITERATURE REVIEW: There is no study for comparison of the efficacy between lidocaine and bupivacaine for the medial branch block. MATERIALS AND METHODS: From August 2011 to May 2013, 186 patients were assigned 0.5% lidocaine(n=136) or 0.25% bupivacaine(n=45) for the ultrasound-guided lumbar medial branch block. All procedures have been performed by the same operator, and 23G, 10 cm needle was placed and drug was injected under ultrasound guide. To target medial branches from lumbar spinal nerve, the groove at the root of transverse process and the base of superior articular process has been identified on transverse scan. Patients were evaluated by pre- and post-interventional(1 hour) Visual Analog Scale and analyzed statistically. RESULTS: Reduction of VAS score in bupivacaine group is significantly greater than that in lidocaine group and post-interventional VAS score in bupivacaine group is significantly lower than that in lidocaine group through analysis of covariance test with adjusted pre-interventional VAS score. In multivariate analysis, while age, sex and treatment level were not significant factors, pre-interventional VAS score and the kind of drug were significant factors. Severe pain before treatment and bupivacaine was indicator of better result. Bupivacaine group reduced pain score in the VAS 2.285 more than lidocaine group with adjustment with other factors. CONCLUSIONS: Bupivacaine is more effective than lidocaine in the reduction of pain after ultrasound-guided lumbar medial branch blocks in posterior facet joint syndrome.

HOW: Referring to pilot studies and previous research studies, we can choose a proper hypothesis and simplify the studies by using a website or Microsoft Excel sheet that contains formulas for calculating sample size in the beginning stage of the study. MORE: There are numerous formulas for calculating the sample size for complicated statistics and studies, but most studies can use basic calculating methods for sample size calculation.

ABSTRACT

STUDY DESIGN: A case report OBJECTIVES: To report an extremely rare case of the spinal meningioma containing bone. SUMMARY OF LITERATURE REVIEW: Spinal meningiomas represent 16.6-46.7% of the primary spinal tumors and 1 to 5% of them are calcified. Ossification is an extremely uncommon event that complicates the resection surgery. MATERIALS AND METHODS: We experienced a 59-year-old patient who complained of weakness in the lower limbs and gait disturbance. Spinal cord compressing mass was discovered on a MRI at T6 and there was a vertical plate at the posterior side of the mass. Surgical finding showed complete ossification in the dural attachment site of the mass. Though the tumor mass could be excised with the inner layer of the dura mater en masse, more forceful retraction of the spinal cord was unavoidable than the other soft mass resection. RESULTS: The preoperative neurological impairment improved after the surgery and she was able to walk well. CONCLUSIONS: Ossification makes a resection difficult and vulnerable to develop neurological deterioration. But if we could suspect such an ossification through an image test, it would be helpful to make a surgical plan to avert a neurologic complication.

ABSTRACT

PURPOSE: We wanted to compare the accuracy between a newly suggested sonography-guided extramedullary alignment guide system and an ordinary intramedullary alignment guide system for the femoral component alignment in Total Knee Arthroplasty (TKA). MATERIALS AND METHODS: Among the patients who underwent TKA from December 2006 to May 2007, an intramedullary technique was applied in 50 cases (the IM group) and an extramedullary technique was used in 50 cases (the EM group). The femoral component alignment angle was measured and compared postoperatively by using an anteroposterior (AP) radiograph of the knee. RESULTS: The average femoral component alignment angle was 94.96degrees (92~98degrees) in the IM group and 95.36degrees (90~99degrees) in the EM group. Both groups didn't show a significant difference. 45 cases (90%) in the IM group and 43 cases (86%) in the EM group were included in the optimal range of the femoral coronal angle (95degrees+/-2). CONCLUSION: There was no significant difference regarding the accuracy of the sonography-guided extramedullary and intramedullary femoral component alignment guide systems for performing Total Knee Arthroplasty.
